    Link Worker

I am the Links worker for the Practice. I have been with the Practice for 2 years. My job largely involves linking patients to the right contacts in order for them to receive practical support to overcome challenges such as financial, housing and occupational issues. For instance, I may refer patients with anxiety to a counsellor or patients struggling financially to advisors. A large part of my work also involves facilitating application processes as this can often be daunting to face alone. I carry out house visits where necessary, ensuring that patients who prefer to be in an environment that they are more comfortable in are seen. I also organize a weekly walking group every Tuesday morning and Thursday at 11:30am to help individuals with isolation and anxiety problems. Everyone who is interested is most welcome!
